Effective as of September 4, 2024 (the “Closure Date”), Cardiac Cath Lab of Lake Charles, LLP (the “Company”) ceased operating Cardiovascular Outpatient Center of Southwest Louisiana, located at 401 Dr. Michael DeBakey Drive, Suite 300, Lake Charles, LA 70601. Following the Closure Date, the Company will no longer participate in the Medicare Program (Title XVIII of the Social Security Act), and the related agreements with the Secretary of the United States Department of Health and Human Services will be terminated in accordance with the provisions of the Social Security Act. The Medicare Program will not make payment for services furnished to patients after the Closure Date.
